- A valid Indonesian identity card (KTP) or certificate of moving abroad;
- A family register (KK);
- A birth certificate, marriage certificate/book, diploma, or baptism certificate*;
- An Indonesian citizenship certificate for Foreign Nationals who acquire Indonesian citizenship or submitting the certificate of citizenship in accordance with the legislation;
- A statement letter of name changes (for those who have changed their name) issued by authorized official;
- Old regular passport.
Note:
*Name, place and date of birth, and parents' name must be listed in the documents. Otherwise, a statement issued by authorized official may be attached.
Additional requirements for applicants of lost passport replacement due to force majeure:
- An application letter of lost passport replacement to the Head of the Immigration Office consisting of name, place and date of birth, domicile address, occupation, and reason of the application;
- A statement letter from the sub-district/authorized authority in accordance with the domicile of the applicant stating that the applicant has experienced force majeure.
